Role of the Primary Care Physician:The Primary Care Physician (PCP) assumes a pivotal role in our collaborative care environment. We emphasize quality care for the patients we serve.
Our multidisciplinary care team includes doctors, advanced practice professionals, pharmacists, care coach nurses, medical assistants, behavioral health specialists, quality coders, referral coordinators, and more.

This extensive support allows our PCPs to manage fewer patients, thereby dedicating more time to each individual.
Key Responsibilities:
- Assess and treat patients in alignment with established care standards.
- Monitor patient care quality and adherence to medical standards using available data and chart reviews.
- Facilitate the coordination of patient services, including specialty referrals and home health care.
- Actively participate in daily huddles as a key medical expert within the care team.
- Collaborate with the regional medical director and center administrator to foster a cooperative practice environment.
- Complete medical documentation promptly, working with quality coders to enhance coding accuracy.
- Adhere to policies and protocols set by Clinical Leadership.
- Engage with the RMD regarding care quality, outcome data, and policy matters.
- Explore growth opportunities for new or existing services within the center.
- Participate in the local primary care on-call program as necessary.
- Ensure compliance with licensing and accreditation requirements.
- Devote 100% of your time to direct patient care, including patient interactions and related administrative tasks.
- Current and unrestricted medical license or willingness to obtain necessary licenses.
- Graduate of an accredited MD or DO program.
- Strong verbal and written communication skills.
- Exceptional interpersonal skills and the ability to communicate effectively with colleagues and patients.
- Commitment to the integrated team-based care model.
- Willingness to adapt to a value-based care environment.
- Outstanding patient service skills.
- Basic computer proficiency, including email and electronic medical records.
- This role involves patient interaction and is subject to TB screening.
- Board certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Active and unrestricted DEA license.
- Medicare Provider Number.
- Two to five years of relevant experience preferred.
- Experience managing a Medicare Advantage patient panel.
- Familiarity with Medicare guidelines and HEDIS quality indicators.
- Guaranteed base salary with bi-annual incentives.

Compensation Range:
The compensation range reflects a good faith estimate of starting base pay for full-time employment. Individual pay may vary based on geographic location and relevant experience, skills, and education.
$199,400 - $298,300 per year
Bonus Incentive Plan:
This position is eligible for a bonus incentive plan based on individual and company performance.
