About the Role

Aveanna Healthcare is seeking a compassionate and skilled LPN to join our team as a Private Duty Nurse. As a Private Duty Nurse, you will provide one-on-one care to our patients in their homes, helping them to recover from illness or injury and maintain their independence.

Responsibilities
  • Implement physician-prescribed plans of care for patients
  • Conduct patient assessments and coordinate care with other healthcare professionals
  • Administer prescribed medications, treatments, and therapies as needed
  • Document care provided in a timely and compliant manner
  • Provide health education and training to patients and their families
  • Accept and write verbal orders from physicians as required, and obtain co-signature from Case Manager
  • Follow up with patients, families, and healthcare professionals on patient concerns and changes in condition
Requirements
  • Active, unencumbered LPN license in the state of practice
  • Current CPR certification from American Red Cross or American Heart Association
  • TB skin test (current within last 12 months)
  • Six months prior hands-on nursing experience preferred
  • Reliable transportation
