As a Licensed Practical Nurse (LPN) with Phoebe Health, you will provide one-on-one skilled nursing care and treatment to patients in their home. Your primary responsibility will be to assess patient needs and develop a comprehensive care plan with the physician.
Key Responsibilities:- Provide skilled nursing care and treatment to patients in their home
- Assess patient needs and develop a care plan with the physician
- Carry out the care plan with patient, family, and support staff
- Monitor and evaluate patient response to treatment, medication, and healing or mobilization progress
- Oversee case management and use a comprehensive and patient-specific care plan with innovative solutions
- Vocational / Technical Degree in Graduate of an accredited school of Practical Nursing
- 1 - 2 years Practical Nursing Experience
- Current Georgia license as a Licensed Practical Nurse (LPN)
- Basic Life Support (BLS) certification
- Valid Georgia Driver's License
- Advanced Cardiac Life Support (ACLS) certification
- Near normal vision - Clarity of vision (both near and far), ability to distinguish colors
- Good manual dexterity and eye-hand-foot coordination
- Ability to perform repetitive tasks/motion
