Job Summary

We are seeking a highly skilled and compassionate Clinical Care Coordinator to join our team at Southeastern Integrated Care LLC. As a key member of our staff, you will play a vital role in providing exceptional care to individuals with mental health and developmental disabilities.

Key Responsibilities
  • Develop and Implement Person-Centered Plans: Collaborate with clients, families, and healthcare professionals to create and update personalized care plans that address unique needs and goals.
  • Advocate and Support: Act as a strong advocate for clients, ensuring their rights and dignity are respected, and providing emotional support and guidance throughout their care journey.
  • Coordinate Services: Work closely with healthcare providers, community resources, and other stakeholders to ensure seamless coordination of services and effective care delivery.
  • Document and Communicate: Maintain accurate and detailed records of client interactions, progress, and outcomes, and communicate effectively with clients, families, and healthcare professionals.
  • Supervise and Train: Provide guidance and support to junior staff members, promoting a culture of excellence and continuous learning.
Requirements
  • Education and Experience: Bachelor's degree in a human services field or related field, with a minimum of 2 years of experience working with mental health and developmental disabilities populations.
  • Skills and Abilities: Excellent communication and documentation skills, strong advocacy and negotiation skills, ability to work independently and as part of a team, and a commitment to providing person-centered care.
Work Environment

This role requires the ability to work in a fast-paced, dynamic environment, with frequent travel to client homes and community settings. A valid driver's license and reliable transportation are required.
