Key Responsibilities- Develop and Implement Person-Centered Plans: Collaborate with clients, families, and healthcare professionals to create personalized plans that address their unique needs and goals.
- Provide Direct Support: Offer emotional support, guidance, and assistance to individuals in their daily lives, helping them to achieve their full potential.
- Coordinate Services: Work closely with healthcare providers, community resources, and other stakeholders to ensure seamless delivery of services and support.
- Advocate and Educate: Promote the rights and dignity of individuals, providing education and support to families and caregivers.
- Document and Report: Maintain accurate records and reports, ensuring compliance with regulatory requirements and company policies.
- Education and Experience: Bachelor's degree in a human services field or related field, with a minimum of 2 years of experience working with mental health and developmental disabilities.
- Skills and Abilities: Excellent communication and documentation skills, ability to work independently and as part of a team, and a strong commitment to providing person-centered care.
- Certifications and Licenses: Valid NC driver's license and current license or certification in a relevant field (if applicable).
This role requires working in a variety of settings, including clients' homes and community locations. You must be able to lift up to 20 pounds and occasionally lift up to 50 pounds. A valid driver's license and reliable transportation are required.
