Job Summary:

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team in providing high-quality home health care services in the Tacoma area. As an LPN, you will play a vital role in delivering patient-centered care, promoting independence, and enhancing the overall quality of life for our clients.

Responsibilities:
  • Assess patient needs and develop personalized care plans under the direction of a registered nurse or licensed physician
  • Provide direct nursing care, including administering medications, performing treatments, and assisting with self-care activities
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
  • Maintain accurate and detailed records of patient progress and outcomes
Requirements:
  • Minimum one year of home health experience (National Agreement waiver applies to current KP Coalition employees)
  • Graduate of accredited LPN program and active Washington State Practical Nurse License
  • Basic Life Support certification and valid driver's license
  • Excellent interpersonal, oral, and written communication skills
  • Ability to work independently and as part of a team
Preferred Qualifications:
  • Two years of home health experience
  • Associates degree

Estimated Salary Range:

$85,000 - $110,000 per year, depending on experience and qualifications
