About the Role

We are seeking a dedicated and compassionate Care Manager to join our team at Sunrise Senior Living. As a Care Manager, you will play a vital role in providing exceptional care and services to our residents and their families.

Responsibilities
  • Build meaningful relationships with residents and their families, providing assistance with activities of daily living and attending to individual care needs.
  • Develop and implement Individualized Service Plans (ISPs) to ensure the unique needs and preferences of each resident are met.
  • Participate in the development of care plans and collaborate with the community team to ensure compliance with OSHA requirements and promotion of Risk Management programs and policies.
  • Respond to the dining needs of residents and guests, maintaining Sunrise hospitality and service standards.
  • Invite, Encourage, and Assist (IEA) residents in life skills and other life-enriching activities as indicated on their ISP and demographic profile.
Qualifications
  • Dedication to serving seniors with excellent customer service skills.
  • Positive attitude, flexibility, and ability to work in a team environment.
  • High School diploma/GED accepted and may be required per state regulations.
  • Previous experience working with seniors preferred.
  • Ability to make choices, decisions, and act in the resident's best interest.
  • Competent in organizational and time management skills.
  • Demonstrate good judgment, problem-solving, and decision-making skills.
