The Physician Leader - Hospitalist/Extensivist will be responsible for providing high-quality medical care to senior patients across multiple settings, including skilled nursing facilities and patient residences. This role requires strong leadership and communication skills, as well as the ability to work effectively with interdisciplinary teams.
Key Responsibilities- Medical Leadership: Provide medical direction and oversight to care teams, ensuring that patients receive high-quality, patient-centered care.
- Care Coordination: Coordinate care with community physicians, Alignment care managers, and other healthcare professionals to ensure smooth transitions between care settings.
- Patient Engagement: See, treat, and manage patients in skilled nursing facilities and patient residences, with a focus on high-risk, frail populations.
- Team Collaboration: Work closely with other clinicians, case managers, and care teams to ensure that all patients receive comprehensive and coordinated care.
- Quality Improvement: Participate in quality improvement initiatives to enhance care delivery systems and improve patient outcomes.
- Administrative Duties: Perform other administrative duties as deemed necessary, including travel to Alignment Healthcare USA, LLC corporate office as needed.
- Education: Medical Doctorate (M.D.) or Doctor of Osteopathic Medicine (D.O.); Internal Medicine or Family Medicine Residency.
- Experience: 2-3 years of Hospitalist, Family Practice, or Internist experience preferred, but will also consider graduating residents and fellows.
- Licensure: Medical License in the state in which you are applying; DEA license, must be board certified or board eligible in specialty.
- Language: Spanish speaking preferred.
$275K-$300K
