Care Coordination Manager

Our Hospice of South Central Indiana is seeking a skilled Care Coordination Manager to join our team. As a key member of our healthcare services team, you will be responsible for overseeing the strategic planning, growth, and administration of our Personal Care Program. This includes supervising staff, recruiting and retaining top talent, developing and implementing program policies and procedures, and working closely with our Director of Business Development to ensure long-term success.
Key Responsibilities:
- Establish and maintain good working relationships with referral sources, including physicians, hospitals, nursing homes, and assisted living facilities.
- Act as liaison between referral sources and operations of the program, providing feedback to ensure optimal customer satisfaction and outcomes.
- Ensure data collection to track department goals and implement improvements as needed.
- Assure that all tracking of community contacts is properly documented in the database.
- Participate in routine meetings to review progress towards goals and opportunities for change and improvement.
- Assist with the development and adherence of the department budget.
- Monitor human resource allocation to ensure the department is within FTE allocations.
- Monitor all requests for resources and ensure department use of items is fiscally appropriate.
- Ensure department activities meet compliance with all state and federal regulations.
- Educate staff on compliance with policies and procedures and department standards.
- Interview, hire, and ensure adequate orientation of qualified candidates.
- Allow for staff recognition, provide timely corrective action, and implement educational plans to maintain standards set for the team and organization.
- Lead and direct all care management activities, ensuring patient care coordination and adherence to protocols and regulatory standards.
- Supervise a multidisciplinary team of Personal Care Aides responsible for home care.
- Develop and maintain a tracking system for seamless care coordination across all services, including transitions and referrals.
- Lead regular team meetings and departmental meetings.
- Collaborate with the Director of Business Development to prioritize staff, needs, and program.
Requirements:
- Bachelor's Degree preferred, Associate's Degree required or lieu of experience.
- Experience in healthcare or post-acute care preferred.
- Experience in management preferred, but not required.
- Valid Driver's License and active auto insurance.
