About the Role:

The Care Coordination Nurse is a vital member of our team at Nationwide Children's Hospital, responsible for ensuring the provision of quality patient care in the appropriate setting through care coordination, case management, utilization management of inpatient admissions, and transitions of care to different levels of care.

In collaboration with the attending practitioner, the family, and other members of the health care team, the Care Coordination Nurse coordinates the individualized plan of care and communicates the plan to appropriate stakeholders.

The Care Coordination Nurse ensures that high-quality care is provided as efficiently and cost-effectively as possible. The nurse facilitates continuity of care by coordinating transitions to post-discharge care.

Responsibilities:

  • Screening and identification of patients
  • Assessment of patient/family strengths and needs
  • Development of a family-centered plan of care
  • Facilitate communication and collaboration amongst the health care team
  • Develop patients' self-management skills through education and resource provision
  • Manage transitions between settings, caregivers, and providers
  • Provide monitoring and follow-up across the continuum of care
  • Maintains knowledge of current concepts, research needs, and research strategies related to care coordination, including case management, utilization review, and discharge planning.
  • Assists in educating others regarding the role of care coordination/case management
  • Participates in ongoing education related to information technology requirements for the department

Qualifications:

  • BSN Required
  • Current licensure as a registered nurse in the state of Ohio is required
  • Valid Ohio driver's license and proof of auto insurance as required by hospital policy

Certifications:

  • Pediatric certification, Advanced-Continuity of Care Certification (A-CCC), or other Case Management certification desirable

Skills:

  • Ability to communicate with health care professionals, by demonstrating diplomacy, tact, and a professional demeanor
  • Working knowledge of CMS and other review agency standards
  • Demonstrated ability to effectively collaborate and delegate tasks
  • Ability to relate to diverse age and demographic backgrounds
  • Computer literate. Working knowledge of MS Office
  • Professionally appropriate analytical and organizational skills required

Experience:

  • Five years of nursing experience
  • At least one year in pediatric nursing is highly preferred
  • Nursing experience may include inpatient nursing, case management, utilization review, discharge planning, community health nursing
