About the Role:

We are seeking a highly skilled Health Systems Specialist 2 to join our team at New York State. As a key member of our Capital District Regional Office, you will play a critical role in ensuring the highest quality of care for our elderly population.

Key Responsibilities:

  • Conduct onsite surveys and assessments to evaluate State and Federal regulatory compliance in skilled nursing facilities and other healthcare settings.
  • Meet with social work teams to discuss compliance and quality of care, and provide feedback for necessary improvements.
  • Prepare detailed reports and participate in reviews to drive quality of life improvements for our residents.
  • Collaborate with interdisciplinary teams to develop and implement strategies for continuous quality improvement.

Requirements:

  • Possession of a New York State license and current registration as a licensed master social worker (LMSW), or licensed clinical social worker.
  • Two years of post-LMSW license experience providing clinical services in a licensed healthcare facility or program, or utilizing clinical knowledge in the review and oversight of clinical services for quality assurance or reimbursement purposes.
  • An Associates degree in a related field can substitute for two years of specialized experience; a Bachelor's degree in a related field can substitute for four years of experience; a master's degree in a related field can substitute for five years of experience; and a doctorate degree in a related field can substitute for six years of experience.

Preferred Qualifications:

  • Experience with providing case management services in a skilled nursing facility or care management along the long-term care continuum.
  • A strong dedication and desire to impact the quality of life for the aged and aging.
  • Excellent communication skills, both oral and written.
  • Federal Surveyor Minimum Quality Test certification and investigatory experience is a plus.
