At Optum, we're transforming care delivery with innovative and personal care. As a Nurse Practitioner or Physician Assistant on our HouseCalls team, you'll play a vital role in providing annual clinical assessments to patients in the comfort of their homes.

Key Responsibilities:

  • Conduct in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing as appropriate.
  • Identify diagnoses to inform care management and active medical management.
  • Communicate findings to inform members' primary care physicians of potential gaps in care.
  • Intervene in urgent and emergent situations as needed.
  • Educate members on disease processes, medication, and compliance.
  • Travel up to 85% of the time.

Requirements:

  • Active NP or PA license, or ability to obtain by start date.
  • Active national certification in NP or PA, or ability to obtain by start date.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions.
  • Reliable transportation for home visits.
