JOB SUMMARY
Under the guidance of a registered nurse, the Practical Nurse Specialist plays a vital role in implementing the nursing process (assessment, planning, execution, evaluation) in Home Health settings, adhering to established policies and protocols. This position involves supporting the registered nurse in executing care plans for patients across all life stages, including adults and seniors, in accordance with recognized policies, procedures, and nursing standards. The role encompasses delivering nursing care to individuals at various life stages, based on expertise and competency assessments.
PRIMARY JOB DUTIES
1. Assisting in the assessment, planning, execution, and evaluation processes tailored to the patient's age and medical condition.
2. Collaborating effectively with a multidisciplinary team to manage patient care efficiently.
3. Communicating pertinent information in line with agency policies and discipline-specific guidelines.
4. Contributing to the overall effectiveness of the program.
5. Organizing and executing tasks efficiently and effectively.
6. Adjusting schedules to optimize agency performance.
7. Upholding the agency's core values in daily operations.
8. Fostering positive interpersonal relationships with all agency members.
9. Promoting and maintaining customer satisfaction.
10. Consistently embodying the mission, vision, and values of the agency.
JOB SPECIFICATIONS
1. Education: Completion of an accredited or approved practical nursing program.
2. Licensure / Certifications: Valid license to practice practical nursing in the relevant state. CPR certification is mandatory.
3. Experience: At least one year of practical nursing experience is required, with a preference for home health experience.
4. Essential Technical / Motor Skills: Proficient hand/eye coordination for administering injections, operating computers, etc. Must possess the ability to communicate effectively and be literate in English. Capable of operating patient care equipment and safely transferring patients.
5. Interpersonal Skills: Ability to cultivate positive interactions with patients, their families, physicians, and staff to ensure effective patient care.
6. Essential Physical Requirements: Capability to lift and/or maneuver objects weighing at least 50 pounds during patient care activities. Frequent lifting, moving, and positioning of patients is required, along with significant physical exertion.
7. Essential Mental Abilities: Competence in assessing patient conditions, formulating care plans, selecting appropriate interventions, and evaluating patient responses to care. Requires strong cognitive skills, including short- and long-term memory, prioritization, and the ability to learn new patient care techniques.
8. Essential Sensory Requirements: Ability to visually assess patients and utilize sight for implementing and evaluating care plans. Hearing is essential for auscultating sounds and effective communication with patients, families, and staff.
9. Exposure to Hazards: Potential exposure to noise, bloodborne pathogens, body fluids, and other hazards during home visits. May encounter situations posing risks to personal safety.
10. Hours of Work: Variable schedule, including weekdays, weekends, and holidays as necessary.
11. Population Served: Care for adolescents, adults, seniors, and pediatric patients.
12. Must possess a valid driver's license and have access to a reliable vehicle.
