About the Position

This Bilingual Mental Health Outreach Worker role is an exciting opportunity to make a real difference in the lives of isolated older adults. You will be responsible for conducting outreach activities, assessing mental health needs, and providing short-term support. Responsibilities:

  • Conduct outreach activities to engage with isolated older adults
  • Assess mental health needs and develop care plans to address them
  • Provide short-term support through home visits and follow-up assessments
  • Collaborate with other providers and build relationships with clients to best assess their long-term support needs

What We Are Looking For:

  • Bachelor's degree in human services plus 1 (one) year experience OR Associate's degree plus 2 (two) years of experience working with behavioral health consumers in an outreach or community based setting
  • Bilingual Spanish required
  • Ability to build positive relationships with isolated older adults
  • Proficient in the use of computer systems, including but not limited to, Windows based software application including but not limited to Word, Excel, PPT and Outlook
  • Ability to process information accurately
  • Professional demeanor and ability to handle confidential materials with discretion
  • Ability to work both independently and as part of a team
  • Must have reliable transportation and valid driver's license in good standing
