Job Summary

We are seeking a highly skilled Transplant Nurse Coordinator to join our team at Northwestern Memorial Healthcare. The successful candidate will be responsible for coordinating comprehensive care for patients undergoing lung transplants, working collaboratively with the interdisciplinary team to ensure seamless care delivery.

Key Responsibilities
  • Coordinate Patient Care: Plan, direct, and provide appropriate care for patients in the designated care setting, ensuring timely and effective treatment.
  • Collaborate with Interdisciplinary Team: Work closely with the healthcare team to coordinate ongoing care, focusing on cost-effective ways to maximize wellness and enhance patients' lives.
  • Develop and Provide Education: Develop, coordinate, and provide education for patients, family, caregivers, and staff based on personalized needs, serving as a resource for ongoing education.
  • Communicate Effectively: Serve as a communication link between the patient, family, caregivers, interdisciplinary team, and outside hospitals/agencies, ensuring clear and timely communication.
  • Contribute to Quality Initiatives: Participate in department, hospital, or other professional committees and/or quality or process improvement initiatives, contributing to the development of best practices.
Requirements
  • Education: Bachelor of Science in Nursing (BSN)
  • Licensure: Licensed Registered Nurse (RN) in the state of Illinois
  • Certifications: CPR-Basic Life Support (BLS)
  • Experience: Minimum 2-3 years of recent and related clinical experience
Preferred Qualifications
  • Specialty Certification: Specialty certification in the clinical area of focus
  • Transplant Experience: Previous transplant or organ-specific experience (i.e., hepatology, nephrology, hematologic malignancies, hematopoietic stem cell transplant, cellular therapies)
