Job Summary
We are seeking a highly skilled and experienced RN Kidney Transplant Coordinator to join our team at Allina Health. As a key member of our transplant program, you will play a critical role in managing the care of patients with organ failure and transplant candidates.
Key Responsibilities
- Coordinate patient assessment, referral, and intake processes to ensure seamless care transitions.
- Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans.
- Provide education and support to patients and families to promote self-care and adherence to treatment plans.
- Develop and maintain policies, procedures, and systems of care to ensure high-quality patient outcomes.
- Participate in quality improvement initiatives to enhance patient care and satisfaction.
Requirements
- Bachelor's degree in Nursing from an accredited institution.
- 2-5 years of experience in clinical management of patients with organ failure/transplant candidates.
- Current Minnesota Registered Nurse license and Basic Life Support certification.
- Preferred: Master's degree and certification in a clinical specialty.
Physical Demands
- Light work with occasional lifting of up to 20 pounds and frequent lifting of up to 10 pounds.
