Job Summary

We are seeking a skilled and compassionate RN Patient Care Manager to join our team. As a key member of our hospice care team, you will be responsible for managing the day-to-day operations of our hospice sites, ensuring the provision of high-quality care to patients, and overseeing the clinical and administrative aspects of our program.

Key Responsibilities
  • Manage and oversee clinical and administrative functions at our hospice sites
  • Respond to phone referrals and inquiries in a timely and professional manner
  • Participate in on-call nursing schedule and provide direct skilled bedside nursing care in patients' homes
  • Manage the site in the absence of the Executive Director or Senior Patient Care Manager
Requirements
  • Registered Nurse with a minimum of Associate's Degree in Nursing required
  • Three years' experience in direct patient care in a clinical setting required
  • Minimum one year of management/supervisory experience required
  • Current license to practice nursing in the state of employment required
