As a Patient Retention Specialist at DaVita, you will play a crucial role in ensuring the retention of home patients by mitigating avoidable clinical losses within a specific division.
Your responsibilities will include:
- Monitoring high-risk patients at the facility, regional, and divisional levels.
- Collaborating with interdisciplinary team members to create tailored care plans.
- Implementing interventions for patients identified as high-risk.
- Providing training and education to support retention strategies for Home teammates.
In this position, you will oversee a patient population of approximately 500 individuals, utilizing data analytics and reporting tools to pinpoint those at greatest risk of losing their treatment modality or facing hospitalization. Your success will be evaluated based on improvements in home modality loss rates and other key clinical metrics, such as peritonitis rates and AI Loss.
Qualifications:
• Active RN license in your state of practice.
• Preferred Bachelor's degree.
• Minimum of three years of experience in nephrology within a PD/HHD environment.
• CNN or CPDN, Bonent certification preferred within two years of employment.
• Valid driver's license and insurable driving record required.
