The Compass Health Network is seeking a highly skilled Behavioral Health Navigator to join our team. As a key member of our integration team, the Behavioral Health Navigator will play a critical role in coordinating care for individuals experiencing behavioral health crises.

Key Responsibilities:


• Collaborate with community behavioral health providers and entities to ensure seamless transitions of care for individuals with behavioral health needs

• Provide consultation on behavioral health issues to jail personnel, promoting a culture of understanding and support

• Assess individuals with behavioral health needs and provide de-escalation support as needed

• Facilitate access to needed resources and services for individuals with behavioral health needs, both during and after their jail stay

• Assist jails in navigating the inpatient hospitalization process for individuals who present a likelihood of serious harm due to mental disorder

• Support individuals with behavioral health needs in accessing behavioral health resources and services

• Participate in critical incident de-briefs for jail staff, promoting a culture of learning and growth

• Maintain accurate and timely documentation of all activities and interactions, as required by the Behavioral Health Liaison Supervisor

• Serve as a member of the multi-disciplinary team, working collaboratively to achieve optimal outcomes for individuals with behavioral health needs

Requirements:


• Bachelor's degree in a related field, such as social work, psychology, or counseling

• Minimum 2 years of experience working in a behavioral health setting, preferably in a jail or correctional facility

• Strong understanding of behavioral health issues and the needs of individuals with mental illness

• Excellent communication and interpersonal skills, with the ability to build strong relationships with jail staff, community providers, and individuals with behavioral health needs

• Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities with ease

• Strong analytical and problem-solving skills, with the ability to think critically and creatively
