Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in Menominee for PRN day visits. As a member of our home care team, you will provide high-quality clinical services to patients in their homes, working under the direct supervision of a Registered Nurse.

Key Responsibilities
  • Provide direct nursing care as prescribed by the physician to acutely ill and/or chronically ill patients in their homes.
  • Record clinical and progress notes, and coordinate other patient care activities/services to provide the highest quality of patient care.
  • Participate in the agency's care coordination process, including timely reporting of patients' needs and changes in condition.
  • Ensure confidentiality of patient health information per agency policy.
Requirements
  • Current LPN licensure in the state of practice.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification.
