LPN Visit Nurse

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in Menominee for PRN day visits. As a valued member of our home care team, you will provide high-quality clinical services to patients in their homes, working under the direct supervision of a Registered Nurse.
Key Responsibilities- Provide direct nursing care to patients in their homes, as prescribed by physicians, and record clinical and progress notes.
- Coordinate patient care activities and services to ensure the highest quality of patient care.
- Participate in the agency's care coordination process, including timely reporting of patient needs and changes in condition.
- Ensure confidentiality of patient health information and maintain accurate records.
- Current LPN licensure in the state of practice.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification.
