Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in Menominee for PRN day visits. As a LPN, you will provide high-quality clinical services to patients in their homes, working under the direct supervision of a Registered Nurse.
Key Responsibilities- Provide direct nursing care to patients in their homes, following the plan of care and in coordination with other healthcare team members.
- Document patient visits thoroughly and accurately in the electronic medical record, adhering to regulatory and payer requirements.
- Participate in the agency's care coordination process, including timely reporting of patient needs and changes in condition, attending patient case conferences, and communicating effectively with patients, families, physicians, and other clinicians.
- Ensure confidentiality of patient health information and maintain a professional demeanor at all times.
- Current LPN licensure in the state of practice.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification.
