LPN Visit Nurse

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in Menominee for PRN day visits. As a LPN, you will provide high-quality clinical services to patients in their homes, working under the direct supervision of a Registered Nurse and/or designated supervisor.
Responsibilities- Provide direct nursing care as prescribed by the physician to acutely ill, and/or chronically ill patients in their homes.
- Record clinical and progress notes, and coordinate other patient care activities/services to provide the highest quality of patient care.
- Participate in the agency's care coordination process, including timely reporting patients' needs and changes in condition, attending patient case conference, and communicating effectively with the patient, family, physician, other clinicians, agency management staff, and case coordinator.
- Current LPN licensure in the state of practice.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification required.
