Job Overview

Harc Inc is seeking a skilled Nurse Coordinator to join our team. This role will be responsible for coordinating and monitoring healthcare services for individuals with intellectual and developmental disabilities living in the community.

The successful candidate will provide caring support, enabling residents to grow and reach their goals. The position requires effective communication and collaboration with staff, ensuring seamless care delivery.

About the Role

This Nurse Coordinator position offers an estimated salary of $73,500 - $85,000 per annum, commensurate with experience and qualifications. We offer a range of benefits, including competitive compensation, comprehensive health insurance, paid time off, and professional development opportunities.

The ideal candidate will hold a current, valid Connecticut Registered Nurse license in good standing, with at least 1 year of experience working with individuals with intellectual/developmental disabilities or a passion to learn and work in this area. Strong Microsoft Office skills, excellent communication, and interpersonal skills are essential for success in this role.

Responsibilities
  1. Coordinate healthcare services based on individual needs, adhering to DDS regulations and policies.
  2. Create and coordinate comprehensive individualized nursing care plans for residents.
  3. Advocate and educate those under their care, promoting independence and goal achievement.
  4. Complete medical documentation, including assessments, referrals, and quarterly audits, while ensuring adherence to best practices and compliance requirements.
  5. Directly manage medical services provided to participants in Harc's programs, maintaining accurate records and reporting.
  6. Develop and deliver training for staff in health-related topics, fostering a culture of knowledge sharing and excellence.
  7. Collaborate with multidisciplinary teams to ensure cohesive care delivery, addressing resident needs through proactive strategies and interventions.
  8. Conduct regular medication audits, identifying areas for improvement and implementing evidence-based solutions.
Requirements
  • Holds a current, valid Connecticut Registered Nurse license in good standing.
  • At least 1 year experience working with individuals with intellectual/developmental disabilities or a passion to learn and work in this area.
  • Valid driver's license and reliable vehicle for travel and work assignments.
  • Competence in Microsoft Word and Excel, with ability to format and print documents.
  • Demonstrates strong communication and interpersonal skills, effectively interacting with diverse stakeholders.
