Job Title: Medical Coordinator RN

About the Role:

Harc, Inc. is seeking a highly skilled and compassionate Registered Nurse to serve as a Medical Coordinator in our team. As a key member of our healthcare services, you will be responsible for coordinating and monitoring healthcare services for individuals with intellectual and/or developmental disabilities living in the community.

This role requires a strong background in nursing, excellent communication skills, and the ability to work effectively with a diverse group of individuals. If you are passionate about providing high-quality care and support, we encourage you to apply.

Salary: $65,000 - $75,000 per year

Responsibilities:

  • Coordinate health care services based on needs for the individual under the direction of DDS regulations and policies.
  • Create comprehensive individualized nursing care plans for residents.
  • Advocate and educate those under their care.
  • Complete medical documentation, including but not limited to, Nursing Assessments, Self-Medication Assessments, referrals for consultation, Nursing Quarterly Audits, and Medication Administration oversight.
  • Manage medical services provided to participants in Harc's programs.
  • Conduct training for staff in health-related topics at New Employee Orientation, annual training courses and as necessary.
  • Potentially Function as an endorsed medication administration instructor for Harc and teaches the Medication Certification course. Retrains staff who have failed the re-certification process and sanctions staff with multiple errors.
  • Delegate initial and annual nursing procedures to staff and determines if further training or re-delegation is needed. Observes medication certified staff during medication passes.
  • Provide direct hands-on clinical support and assessments as necessary within DDS regulations while adhering to Nursing Best Practices and Standards of Care.
  • Complete medication audits per DDS regulations.

Requirements:

  • Holds a current, valid Connecticut Registered Nurse license in good standing
  • Has, at minimum, an associate's degree in nursing.
  • At least 1 year experience working with individuals with Intellectual/developmental disabilities or a passion to learn and work in this area.
  • Valid driver's license and reliable vehicle to travel and complete work assignments in residential locations
  • Competence in Microsoft Word and Excel, and ability to format and print documents
  • Demonstrates the ability to communicate with persons with intellectual disabilities.
  • Able to communicate with staff clearly and effectively.
  • A variety of lifting and transfer supports for residents may be required to facilitate full independence.
  • Ability to complete significant amounts of computer screen work and data entry without restriction. Detailed oriented and ability to prioritize work assignments, work independently, and effectively manage time and resources.
  • Capability to communicate with staff clearly and effectively.
  • Possess a sincere desire to help with a caring, compassionate, empathetic, and responsible attitude.
