About the Role

We are seeking a highly skilled RN Care Manager to join our team at Intermountain Healthcare. As a key member of our healthcare team, you will play a critical role in improving the health of patients with chronic conditions or complex needs.

Key Responsibilities
  • Collaborate with physicians and other healthcare professionals to develop and implement comprehensive care plans that meet the unique needs of each patient.
  • Provide education and support to patients and their families to help them manage their health and wellness.
  • Identify patients at risk for proactive intervention and develop strategies to prevent hospitalizations and emergency department visits.
  • Coordinate care for complex cases and patients with multiple specialists.
  • Maintain accurate and up-to-date documentation of patient assessments and care plans.
  • Participate in quality improvement initiatives to improve patient outcomes and reduce healthcare costs.
  • Collaborate with payers and outside agencies to promote patient-centered care.
Requirements
  • Associate Degree in Nursing (ADN/ASN) or Bachelor's Degree in Nursing (BSN)
  • Current RN license in the state of practice
  • Minimum of two years of clinical experience in an outpatient setting or two years of experience coaching individuals with complex and/or chronic conditions
Preferred Qualifications
  • Bachelor's Degree in Nursing (BSN)
  • Prior experience in care coordination in a community setting or primary care practice
  • Program development, operations management, and/or supervisory experience
Physical Requirements

This role requires the ability to see and read information, labels, and assess patient needs. The employee must be able to verbally communicate and hear and understand spoken information, alarms, needs, and issues quickly and accurately. Manual dexterity is required to manipulate complex and delicate equipment with precision and accuracy. The employee must be able to lift and utilize full range of movement to transfer patients and walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment.
