Job Summary:

The Registered Nurse RN Case Manager will perform initial assessments in clients' places of residence to determine care needs and develop appropriate plans of care. This role requires strong interpersonal skills, problem-solving abilities, and excellent written and oral communication skills.

Responsibilities:
  • Conduct initial assessments to determine care needs and develop plans of care
  • Revises plans of care as necessary to ensure quality and continuity of care
  • Serve as account executive for assigned clientele, maximizing quality of care and profitability of services
  • Ensure compliance with all required protocols and documentation for various payors
  • Collaborate with care coordination team to ensure best fit for care providers and clients
  • Coordinate services and inform physician and other agencies of client's condition and change of needs
  • Provide on-site training and education to care providers as needed
  • Supervise, teach, and provide clinical direction to other nursing personnel
  • Obtain physician orders as necessary for admissions and ongoing clinical care
  • Track quality metrics, including readmission rates, diagnosis-related recovery trends, and hospitalizations
  • Promote agency philosophy and administrative policies
  • Perform on-call responsibilities and provide on-call services to clients and families as assigned
