Job Summary:The Registered Nurse RN Case Manager will perform initial assessments in clients' places of residence to determine care needs and develop appropriate plans of care. Responsibilities:- Conduct initial assessments to determine care needs and develop plans of care
- Revises plans of care as necessary to ensure quality and continuity of care
- Serve as account executive for assigned clientele, maximizing quality of care and profitability of services
- Ensure compliance with all required protocols and documentation for various payors
- Collaborate with care coordination team to ensure best fit for care providers and clients
- Coordinate services and inform physician and other agencies of client's condition and change of needs
- Provide on-site training and education to care providers as needed
- Supervise, teach, and provide clinical direction to other nursing personnel
- Obtain physician orders as necessary for admissions and ongoing clinical care
- Track quality metrics, including readmission rates, diagnosis-related recovery trends, and hospitalizations
- Promote agency philosophy and administrative policies
- Perform on-call responsibilities and provide on-call services to clients and families as assigned
