We are seeking a skilled RN Care Manager to join our team at Saint Joseph Hospital in Denver, Colorado. As a RN Care Manager, you will work collaboratively with physicians and other healthcare professionals to improve the health outcomes of patients with complex needs.
Key Responsibilities- Perform comprehensive assessments of patients' health needs, including health status, behaviors, level of function, psychosocial situation, and available support systems.
- Establish care plans in collaboration with primary care physicians and the patient care team.
- Provide health education and support to patients and their families.
- Identify patients at risk for proactive intervention and develop strategies to address their needs.
- Coordinate care for complex cases or those patients seeing multiple specialists.
- Maintain accurate and up-to-date documentation of patient assessments and plans.
- Analyze patient care trends and collaborate with the care team to improve overall quality and efficiency of care.
- Associate's Degree in Nursing (ADN/ASN)
- Current RN license in the state of practice
- Minimum of two (2) years of clinical experience in an outpatient setting or a minimum of two (2) years of experience coaching individuals with complex and/or chronic conditions
- Bachelor's Degree (BS/BA/BSN)
- Prior experience in care coordination in a community setting or primary care practice
- Program development, operations management, and/or supervisory experience
This position requires ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies. Frequent interactions with patient care providers, patients, and visitors require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations. Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy is also required.
