Salary and Benefits

Job Description

As a Compassionate Patient Care Specialist, you will have the opportunity to work closely with patients, families, and interdisciplinary teams to deliver high-quality care. Your responsibilities will include:

  1. Performing nursing assessments and collaborating with the nursing team, other disciplines, patients, and families to develop effective plans of care.
  2. Establishing realistic, measurable short and long-term patient goals with interventions put in place as needs arise to minimize rehospitalizations and continued evaluation of outcomes.
  3. Administering medications and performing treatments per physician orders.
  4. Delegating patient care responsibilities to staff and supervising them to ensure that tasks and interventions are implemented.
Required Skills and Qualifications

To be successful in this role, you should possess:

  • A graduate degree from an approved school of nursing and current licensure by the State Board of Nursing.
  • CPR Certification.
  • One to three years of experience working as a RN in a related setting is preferred.
