Job Title: Patient Care Advocate

At Serenity Healthcare, we are seeking a compassionate and skilled Patient Care Advocate to join our team of clinical support staff. As a Patient Care Advocate, you will play a vital role in guiding our patients in taking back their lives by helping them understand their treatment options and providing support throughout their journey.

Key Responsibilities:
  • Act as a liaison between patients and their providers to ensure seamless communication and care coordination.
  • Deliver exceptional face-to-face patient experiences at our clinics, providing a safe and supportive environment for patients to share their concerns and needs.
  • Assist patients in understanding their treatment options and developing personalized care plans that meet their unique needs.
  • Consult with patients to gather pertinent information regarding their current symptoms, mental health status, and medication history.
Requirements:
  • High School Diploma or equivalent required.
  • At least 2+ years of full-time work experience in a service-oriented role.
  • Strong interpersonal and communication skills, with the ability to build trust and rapport with patients and their families.
  • Excellent problem-solving and critical thinking abilities, with a focus on providing patient-centered care.
