About the Role

This is a full-time position located at Lakewood Continuing Care Center. The successful candidate will work 32 hours per week on a rotating schedule, including evenings, nights, and weekends. You will be responsible for providing direct and indirect patient care under the direction of nursing staff, performing duties such as feeding, bathing, dressing, grooming, or moving patients, or changing linens. May transfer or transport patients.

Key Responsibilities

  1. Communicate and interact with patients and families, physicians, peers, and other healthcare members to maximize successful customer outcomes and unit function.
  2. Precept team members to job function
  3. Orient patients to hospital environment and routine
  4. Attend and serve on one unit committee or educational peer opportunity to improve quality and/or engagement.
  5. Document all aspects of patient care in a timely manner.
  6. Participate in unit quality improvement initiatives.
  7. Communicate appropriate information and patient needs to oncoming staff at change of shift and to RN in a timely manner for follow-up.
  8. Provide care in accordance with the patient's plan of care.
  9. Label and Deliver specimens per policy.
  10. Maintain a safe environment complying with NLH policies and procedures; reports and directly addresses environmental hazards and violations of patient safety policy and/or protocol when involved or observed.
  11. May perform other duties upon request.
