Quality LPN Medical Records Specialist

UPMC Hamot is seeking a compassionate and skilled Part-Time Licensed Practical Nurse to join our dynamic 6 South team.
This role offers the opportunity to provide exceptional nursing care to our diverse patient population while working in a supportive and collaborative environment.
The 6 South unit is a 43-bed Medical/Surgical unit with 11 beds dedicated as Step Down to service higher acuity patients.
The Step-Down unit serves patients after kidney transplant and hepatobiliary surgeries along with other patient populations who require closer monitoring as a bridge from ICU to the regular nursing unit.
The unit offers care for patients requiring CPAP, BIPAP, and Optiflow oxygen therapy, glycemic control, and vasoactive medications.
The general telemetry beds offer care of the patient requiring anti-arrhythmic medications, electrolyte replacement, and hemodynamic medication administration.

LPNs act as a member of the health-care team by exercising sound nursing judgment based on preparation, knowledge, experience in nursing and competency and able to provide care and IV therapy following UPMC LPN Practice Guidelines and policies, and the State Board Regulations.
The Licensed Practical Nurse (LPN) participates in the planning, implementation and evaluation of nursing care using focused assessment in settings where nursing takes place under the direction of a Registered Nurse or other licensed UPMC provider.
Collaborate with licensed professional nurses and healthcare team members for guidance when care needs exceed LPN scope or competency, or when a patient's condition changes significantly.
Follow established policies, UPMC LPN Practice Guidelines, and the State Practice Act.
Create a healing environment based on Relationship-Based Care, formulating daily goals and care plans that involve patients in a holistic manner.
Communicate patient conditions to other caregivers and ensure comprehensive documentation to enhance care coordination.
Integrate national professional standards with health system goals to improve patient safety, quality, and satisfaction.
Foster a compassionate, patient-focused experience by building strong relationships with patients, families, and colleagues.
Actively engage in unit activities, support new LPNs, and provide patient education throughout the care journey.
Seek feedback for continuous professional improvement and identify opportunities for clinical quality and workplace enhancement.
Collaborate with the healthcare team to achieve optimal patient outcomes while maintaining a balance between work and personal life.
Graduation from a State Board of Nursing-approved Licensed Practical Nursing Program.
Experience: 0-2 years of nursing experience.
Completion of an IV therapy curriculum within 4 months of hire/transfer.
Ability to demonstrate knowledge of growth and development principles and provide appropriate care based on age-specific needs.
Skilled in identifying process improvement opportunities and collaborating with healthcare teams, patients, and families to achieve optimal outcomes.
Ability to establish positive relationships with executives, managers, physicians, and support staff.
Capable of working effectively in a fast-paced environment with changing priorities and specialized equipment.
Must have mobility and visual manual dexterity, along with the stamina for frequent walking, standing, lifting, and positioning of patients.
Proficient in using electronic health records.
LPN must maintain state licensure where they are practicing or in a state covered by a licensure compact agreement with the state where the facility is located.
Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state.
Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
