Job Summary
As a Licensed Practical Nurse (LPN) - Care Coordinator, you will play a vital role in developing and implementing care plans, managing patient transitions, and coordinating care activities. This is an exciting opportunity to join i4 Search Group Healthcare as a full-time permanent employee.

Key Responsibilities:
- Collaborate with healthcare providers, patients, and families to develop individualized care plans
- Serve as a liaison between patients, families, and healthcare providers to advocate for patient needs and preferences
- Provide education to patients and their families on disease management, medication adherence, and healthy lifestyle choices
- Track patient progress, identify potential issues, and coordinate follow-up care to ensure continuity and quality of care

Requirements:
- Current LPN license in the State of practice
- Minimum of 6 months Long Term Care experience
- Geriatric behavioral health experience preferred
