We are seeking a highly skilled Inpatient Care Coordinator - Social Worker to join our team. This role plays a critical part in ensuring seamless transitions for patients upon discharge, while also providing supportive counseling and advocacy services.

The ideal candidate will have a Master's degree in Social Work from an accredited institution and be licensed as a Social Worker in the state of New Jersey. At least two years of experience working in an acute hospital setting is preferred, along with strong critical thinking skills and the ability to maintain effective relationships with patients, families, and healthcare professionals.

This role requires excellent communication and organizational skills, as well as the ability to work collaboratively as part of a multidisciplinary team. The Inpatient Care Coordinator - Social Worker will be responsible for developing and implementing comprehensive discharge plans that address the psycho-social needs of patients, while also promoting safe and effective transitions to outpatient care settings.

Salary range: $85,000 - $105,000 per year, depending on experience.
