Patient Care Coordinator

Northampton Post Acute in Easton, PA is seeking a dedicated and caring Patient Care Coordinator to join our team. As a Patient Care Coordinator, you will play a critical role in ensuring the comfort, safety, and satisfaction of our residents.
About the Role:
- Coordinate patient care activities, including medication management, wound care, and pain management
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Monitor resident outcomes, identify areas for improvement, and implement strategies to address them
Requirements:
- Active CNA license in the state of PA
- Strong organizational and time management skills
- Ability to work effectively in a fast-paced environment
