We are seeking a skilled Nurse Practitioner to join our team as a Chronic Disease Care Coordinator. In this role, you will work closely with our primary care clinics to improve the quality of chronic disease care for our patients.
Key Responsibilities- Provide direct clinical care to patients with uncontrolled chronic conditions, such as hypertension, diabetes, and asthma.
- Develop and implement patient education plans to promote chronic disease prevention and management.
- Collaborate with healthcare teams to identify and address barriers to care, including socioeconomic and cultural factors.
- Participate in performance improvement initiatives to optimize chronic disease care outcomes.
- Current certification as a Nurse Practitioner in New York State.
- Valid Basic Life Support certification through the American Heart Association.
- Strong computer skills, including proficiency in Microsoft Office and Epic.
