- Collaborate with healthcare providers to identify resources and specialists to meet member needs.
- Provide ongoing follow-up and monitoring of member status, revising care plans as necessary to meet goals and unmet needs.
- Facilitate care management and collaborate with providers to ensure timely access to needed care or services.
- Collect, document, and maintain member information and care management activities to ensure compliance with current state, federal, and third-party payer regulators.
- Bachelor's Degree in nursing and RN licensure required.
- One year of clinical experience required.
- Valid driver's license and proof of car insurance.
- Competitive pay: $54,000.00 - $97,100.00 per year.
