Key Responsibilities:
  • Evaluate member needs, identify barriers to care, and develop personalized care plans to address social determinants of health.
  • Collaborate with healthcare providers to identify resources and specialists to meet member needs.
  • Provide ongoing follow-up and monitoring of member status, revising care plans as necessary to meet goals and unmet needs.
  • Facilitate care management and collaborate with providers to ensure timely access to needed care or services.
  • Collect, document, and maintain member information and care management activities to ensure compliance with current state, federal, and third-party payer regulators.
Requirements:
  • Bachelor's Degree in nursing and RN licensure required.
  • One year of clinical experience required.
  • Valid driver's license and proof of car insurance.
