Role Summary & Key Responsibilities:
The RN Case Manager plays a vital role in supporting the Director of Health and Wellbeing and overseeing the daily clinical functions of the community. This position involves planning, organizing, and coordinating nursing processes to deliver exceptional care to residents, adhering to The Waters' standards of service, presence, and collaboration. The RN Case Manager acts as a primary contact for team members, residents, their representatives, and healthcare providers, ensuring outstanding customer service characterized by courtesy, dignity, and respect.
Clinical Duties:
- Oversees resident care with expertise in assessments, symptom management, crisis intervention, service recovery, and family engagement.
- Collaborates with healthcare professionals to evaluate resident care needs and adjusts service plans accordingly, utilizing an interdisciplinary approach.
- Identifies clinical education requirements beyond standard practices, including specific competencies for new residents.
- Conducts thorough clinical assessments to determine person-centered and holistic care needs, promoting positive outcomes.
- Delegates and supervises tasks assigned to team members effectively.
- Ensures clinical data integrity through adherence to best practice documentation standards and appropriate data management systems.
- Responds promptly to emergencies, including calls from residents, security concerns, and falls.
- Investigates and reports any suspected resident abuse or neglect following established protocols.
- Maintains compliance with HIPAA regulations to protect resident privacy.
- Assists residents and their representatives in understanding the financial implications of care plans, including the risks and benefits of recommended or declined services.
- Participates in auditing nursing practices to ensure compliance with relevant state and federal regulations.
Supervisory Duties:
- In collaboration with the Director of Health and Wellbeing, supervises the health and wellbeing team, providing coaching, counseling, and disciplinary actions to foster professional growth and development.
- Encourages teamwork and a shared purpose among staff delivering a range of services, including daily living assistance, nursing care, and wellness programs.
- Identifies opportunities for team members' professional development.
- Adapts educational initiatives to suit the learning needs of individual team members.
- Maintains risk management protocols, including assessments of medication errors and fall incidents.
- Works with the Director of Health and Wellbeing to implement reporting procedures and interventions, including documentation and internal investigations.
- Ensures the effective application of resident service plans, ensuring thorough and accurate completion of delegated services.
Qualifications:
- Graduate of an accredited nursing program required.
- Current Registered Nurse (RN) license in good standing in the state of employment.
- Preferred two years of nursing experience in a geriatric environment.
- Dementia certification or willingness to obtain certification.
- Preferred prior supervisory and leadership experience.
