About the Role

We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice RN Case Manager. In this role, you will play a critical part in delivering high-quality care to older adults at the end of life, addressing their physical, emotional, social, mental, and spiritual needs.


As a Hospice RN Case Manager, you will work closely with our interdisciplinary team to develop, implement, and evaluate patient care plans, ensuring seamless coordination of services and effective communication with patients, families, and healthcare professionals.


Key responsibilities include:


  • Providing ongoing assessments of patients' physical, psychological, social, and spiritual needs
  • Developing and implementing individualized care plans to maximize patient comfort and quality of life
  • Coordinating hospice services, including medical, nursing, and social work interventions
  • Collaborating with patients, families, and healthcare teams to ensure comprehensive care
  • Documenting patient information accurately and thoroughly

Requirements

To be successful in this role, you will need:


  • A current RN license in the state(s) where you practice
  • A minimum of three years of nursing experience, preferably in critical care, oncology, long-term care, or medical/surgical settings
  • Strong knowledge of pain and symptom management, as well as experience with hospice and home care
  • Excellent interpersonal and communication skills, with the ability to work effectively with patients, families, and healthcare teams
  • Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities
  • Reliable transportation, a valid driver's license, and current auto insurance
