Job Summary

We are seeking a highly skilled and experienced Registered Nurse to join our team as a Patient Placement Coordinator. This role is responsible for coordinating the appropriate placement of patients within the hospital, ensuring that they receive the best possible care.

Key Responsibilities
  • Collaborate with nursing staff and multidisciplinary teams to place patients in the most suitable level of care, taking into account patient acuity, bed availability, and required services.
  • Utilize software systems to document patient movement and initiate processes with other departments, such as case management and registration.
  • Assess and document patient information, ensuring that it is accurate and up-to-date, and that it meets departmental guidelines.
  • Stay informed of changes in managed care and communicate procedural and financial impacts to physicians, as well as alerting case management to any concerns.
  • Evaluate and negotiate patient placement when beds are not available, working in collaboration with administrative supervisors, charge nurses, case management, and physicians.
  • Communicate hospital capacity issues to relevant areas, respond to phone calls quickly, manage call queues, and inform Manager/Director when placement conditions are near alert or crisis status, per guidelines.
  • Work with physicians, patient care service staff, and case managers to ensure timely discharges, escalating bed clean and discharge challenges when necessary.
  • Communicate with the Emergency Department and Surgery staff to prevent a backlog of admissions and implement procedures to prevent a backlog.
  • Participate in projects, data collection, and other activities related to improving bed management.
  • Adhere to the professional nursing standards defined in the professional practice model and use the ANA Code of Ethics for Nurses to guide responses to current and evolving health and nursing needs of patients and patient populations.
  • Work in a constant state of alertness and safe manner.
  • Perform other duties as assigned.
Requirements
  • Graduate of an accredited school of nursing or education equivalency for licensing.
  • Five years of registered nurse experience in an acute care hospital setting.
Physical Requirements
  • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
  • Frequent sitting, standing, walking, reaching, and repetitive foot/leg and hand/arm movements.
  • Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
  • Frequent use of hearing and speech to share information through oral communication, and ability to hear alarms, malfunctioning machinery, etc.
  • Frequent keyboard use/data entry.
  • Occasional bending, stooping, kneeling, squatting, twisting, and gripping.
  • Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
  • Rare climbing.
Required Professional License and/or Certifications
  • Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR).
  • Registered Nurse (RN) Issued by Compact State.
  • Or Registered Nurse (RN) - Missouri Division of Professional Registration.
  • And FUTURE - Registered Professional Nurse (RN) within 6 months - Illinois Department of Financial and Professional Regulation (IDFPR).
  • Or Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR).
  • Registered Nurse (RN) Issued by Compact State.
