Job Summary

We are seeking a highly skilled and experienced Registered Nurse to join our team as a Patient Placement Coordinator. This role will be responsible for coordinating the appropriate placement of patients in our acute care setting, ensuring that patients receive the best possible care and outcomes.

Key Responsibilities
  • Collaborate with nursing staff and multidisciplinary teams to place patients in the most appropriate level of care.
  • Utilize software systems to document patient movement and initiate processes with other departments.
  • Assess and evaluate patient information to determine the best placement options.
  • Communicate effectively with physicians, case managers, and other stakeholders to ensure seamless patient flow.
  • Develop and implement strategies to improve patient placement and reduce bed assignment delays.
Requirements
  • Graduate of an accredited school of nursing.
  • Five years of experience as a Registered Nurse in an acute care setting.
  • Strong communication and collaboration skills.
  • Ability to work in a fast-paced environment and prioritize multiple tasks.
