Job Summary and Qualifications

The Neuroscience Nurse Navigator serves as a vital link between the patient, family, and healthcare team, providing comprehensive guidance and support throughout the care delivery process.

  • Demonstrates expertise in coordinating care, advocating for patients, and facilitating seamless communication among multidisciplinary teams.
  • Utilizes advanced knowledge of neuroscience to educate patients on diagnoses, treatment options, and self-care strategies, promoting informed decision-making and empowerment.

Key Responsibilities:

  • Initial screening and triage via phone, with referrals made as necessary.
  • Completing patient intake forms, including questionnaires and other necessary documentation.
  • Serving as a central point of access for patients, connecting them with relevant care providers and resources.

Required Skills and Qualifications:

  • Bachelor's degree in Nursing (BSN) or Associate Degree in Nursing (ADN)
  • Current RN license in the Commonwealth of Virginia or compact license
  • 3+ years of experience in adult neurosurgical settings, with a basic understanding of neurological patient care
  • Proficiency in computer software applications
