The Clinical Care Coordinator RN is responsible for managing care to evaluate and synchronize all medical and supportive services for complex populations with significant health needs. This role aims to enhance the quality and cost-effectiveness of care provided. The coordinator will create individualized care plans for long-term care members, addressing their needs and educating them and their families about available services and benefits for optimal care.

Key Responsibilities:

  • Monitor emails for urgent communications. Conduct self-scheduled assessments promptly based on leadership reports, ensuring timely documentation of visits. Assess members for potential community relocations. Manage task queues and complete necessary tasks. Follow up as required, collaborating with nursing facility staff to ensure member safety and satisfaction while meeting compliance and contractual obligations. Conduct in-person follow-ups with members and provide updates on ongoing investigations. Participate in scheduled team meetings and complete mandatory training sessions.

Performance Metrics:

  • Complete all assigned tasks, in-person assessments, and mandatory training promptly to ensure member safety and satisfaction while adhering to compliance and contractual standards.
  • Foster and maintain an inclusive environment.

Preferred Background:

  • Experience in home health or roles involving in-home assessments.
  • Previous clinical experience in a nursing facility.

Essential Skills and Qualifications:

  • Basic computer skills and typing proficiency.
  • Strong critical thinking abilities and nursing judgment for health risk assessments to ensure member safety.
  • Flexibility, resilience, and reliable transportation with a valid driver's license (mileage reimbursement available). Excellent communication skills for member advocacy.

Desirable Skills:

  • Bilingual candidates are preferred but not mandatory.
  • Clinical experience in home health or nursing facilities, along with at least one year of hospital experience.

Licensure Requirements:

  • Must possess a current Texas RN license without stipulations. RUG certification will be required upon hiring and is reimbursable.
