We are seeking a highly experienced and strategic leader to drive the success of our care coordination programs system-wide. As the Senior Director of Care Coordination, you will be responsible for developing and implementing strategies to improve clinical and financial outcomes across our organization.

Key Responsibilities

  • Develop and implement care coordination programs to improve health service utilization, quality, and expenses
  • Manage key performance indicators (KPIs) related to health service utilization, quality, expenses, and other critical factors in alignment with value-based care
  • Collaborate with clinical, product, and other teams to identify and implement innovative care management programs
  • Develop and coordinate strategies for ensuring delivery of care in the most appropriate, lowest cost setting environments
  • Assume fiscal responsibility for budgetary planning and implementation for assigned areas
  • Recruit, engage, develop, lead, and manage assigned staff
  • Work in a constant state of alertness and safe manner

Requirements

  • Master's degree
  • Valid RN license in WI, MO, IL, and/or OK
  • Ten years' experience, with five years' in leadership
