About the Role

We are seeking a highly skilled Medical Records Coordinator RN/LVN to join our team at Skyline Nursing Center. As a Medical Records Coordinator, you will play a crucial role in ensuring the accuracy and timeliness of resident care plans and medical records.

Key Responsibilities

  • Collaborate with interdisciplinary teams to assess resident needs and develop effective care plans
  • Ensure compliance with federal and state regulations regarding completion and coordination of the RAI process
  • Maintain accurate and up-to-date documentation of resident information, including medical history, diagnoses, and treatment plans
  • Coordinate and manage daily stand-up meetings to review resident care and set assessment reference dates
  • Develop and maintain strong relationships with facility staff, residents, and families to ensure seamless communication and care
  • Assist with identification of significant changes in resident condition and update MDS and care plans accordingly
  • Prepare scheduling and notices for resident care planning conferences and communicate outcomes to responsible staff and parties
  • Participate in quality assurance meetings and other relevant activities to maintain high-quality care standards
  • Maintain confidentiality of sensitive information and adhere to all HIPAA guidelines

Requirements

  • Degree in Nursing (RN or LVN) from an accredited program and current licensure in the state of practice
  • Minimum 2 years of experience in nursing, preferably in a Skilled Nursing Facility
  • Excellent knowledge of Case-Mix, Federal Medicare PPS process, and Medicaid reimbursement
  • Thorough understanding of Quality Indicator processes and OBRA regulations
  • Experience with MDS 3.0 is preferred
