Job Description

We are seeking a skilled Care Coordinator Specialist to join our team at Grays Adult Activity Home.

As a key member of our healthcare team, the successful candidate will be responsible for assessing individuals' nursing and medical needs. This includes developing personalized plans of care, obtaining physician orders for medication and treatment administration, and ensuring accurate documentation of progress notes.

The ideal candidate will have strong communication skills and be able to work effectively with interdisciplinary teams to provide high-quality care. They will also be responsible for administering medication, providing health education, and maintaining accurate medical records.

Beyond their clinical expertise, our Care Coordinator Specialist must demonstrate excellent time management skills, as they will need to prioritize tasks and manage their workload efficiently. Additionally, they will need to maintain confidentiality and adhere to strict standards of patient privacy.

This is a full-time position, requiring an 8-hour presence on weekdays. If hired alongside an LVN, our RN consultant will provide regular consultation services, documenting their interactions and ensuring seamless care coordination.
