Job Summary

The Methodist Hospital Specialty and Transplant is seeking a skilled RN Case Manager to join our team. As a RN Case Manager, you will work with patients and families to develop a plan that optimizes functional and self-care abilities, facilitates system access and appropriate utilization of services, and promotes quality and cost-effective outcomes.

Responsibilities
  • Assess discharge needs of high-risk patients and identify resources available to address those needs.
  • Coordinate referrals and document plans to ensure seamless transitions of care.
  • Utilize community resources to support patients' post-hospital needs and track resource statistics to identify improvement opportunities.
  • Attend medical staff and other hospital committee meetings to report findings and facilitate education of hospital staff, physicians, patients, and caregivers.
  • Perform admission and continued stay reviews, provide clinical information to payers, and discuss alternate levels of care with physicians.
Requirements

To be successful in this role, you will need:

  • A graduate degree in nursing from an accredited program.
  • A minimum of 3 years of clinical experience in the area of case management.
  • A valid RN license to practice in the state of Texas or an active multi-state RN compact license.
  • Preferred qualifications include a Bachelor of Science in Nursing and experience in utilization review, resource management, discharge planning, or case management.
