RN Case Manager

We are seeking a skilled RN Case Manager to join our team at Methodist Hospital. As a key member of our healthcare team, you will work closely with patients, families, and healthcare professionals to develop and implement effective discharge plans that promote quality, cost-effective outcomes.
Responsibilities- Assess and identify discharge needs of high-risk patients, and develop plans to address these needs.
- Coordinate referrals and document plans to ensure seamless transitions of care.
- Utilize community resources to support patients' post-hospital needs, and document the availability of these resources.
- Perform admission and continued stay reviews, and provide clinical information to payers as needed.
- Facilitate and provide education to hospital staff, physicians, patients, and caregivers to optimize resource utilization.
- Stay current on requirements from JCAHO, HCFA, and other state/federal agencies to ensure provision of high-quality care.
- Track resource statistics to identify areas for improvement, and attend medical staff and other hospital committee meetings to report findings.
- Graduate of an accredited diploma, associates, or baccalaureate degree nursing program.
- Minimum of 3 years clinical experience in the area of case management.
- Employee completes initial and ongoing training and competencies as defined by service line, facility, and department/unit.
- Preferred: At least one year experience in utilization review, resource management, discharge planning, or case management.
- Required: RN License to practice as a professional registered nurse in the state of Texas or Active Multi-State RN Compact License.
- Preferred: Bachelor of Science in Nursing.
