RN Case Manager PRN

We are seeking a skilled RN Case Manager to join our team at Lourdes Health Network. The successful candidate will be responsible for providing high-quality case management services to patients and their families, ensuring seamless transitions and optimal outcomes.
Key Responsibilities- Conduct thorough assessments of patients' needs and develop personalized care plans
- Collaborate with healthcare teams to ensure timely and effective care coordination
- Provide education and support to patients and families to promote self-management and empowerment
- Monitor and evaluate patient outcomes, making adjustments to care plans as needed
- Develop and maintain relationships with community resources and healthcare providers to ensure access to necessary services
- Graduate from an accredited school of nursing (RN) required, bachelors preferred
- Two (2) or more year's clinical experience preferred, with a focus on case management
- Ability to work effectively in a fast-paced environment, prioritizing multiple tasks and deadlines
- Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare teams
- Basic computer skills, with proficiency in electronic medical records and other relevant software
- Case Management Certification (CCM) preferred
- Bilingual and/or English-Spanish speaking abilities preferred
- Ability to work with culturally diverse populations
