About the RoleThe Case Manager RN PRN is responsible for coordinating and directing care specific to the needs of each patient. This includes managing rehabilitative care to ensure optimum outcomes, collaborating with the Rehabilitation Program Director and Facility Case Management Director, and coordinating efforts within the Rehabilitation team.
Responsibilities- Implement patient treatment plans that support the patient's strengths, abilities, needs, and preferences.
- Facilitate patient involvement throughout the rehabilitation process.
- Document findings of Discharge Planning Evaluation (DPE) and psychosocial assessments.
- Communicate social, financial, or discharge needs and preferences of the patient/family/caregiver.
To be successful in this role, you will need:
- Completion of RN Diploma program or related Healthcare program.
- Minimum of 3 years recent acute clinical practice or related healthcare experience.
- Critical thinking, service excellence, and good interpersonal communication skills.
- Ability to read/comprehend written instructions, ability to follow verbal instructions, PC skills, demonstrates analytical and critical thinking abilities with proactive decision-making and negotiation skills.
